The conditions of thyroid and PCOS seem to be happening together more often than either condition is being discussed on its own. You may already be managing polycystic ovary syndrome, now known as PMOS following the 2026 Lancet Commission renaming. If you’ve had a blood test recently that flagged a thyroid issue, you are not imagining a pattern.
Research consistently finds these two conditions overlapping at rates well above chance. At least one shared mechanism connects them. A single visit to a gynaecologist or endocrinologist rarely explains why.
In this post I’ll be looking at what the current evidence says about the relationship between the thyroid and PCOS, why one can quietly make the other worse, and what a food-first approach to both looks like in practice. Nothing here is a replacement for proper testing with your doctor, but it should allow you to ask smarter questions at your next appointment.
What Links Thyroid and PCOS Together
Most women are told to manage PCOS and thyroid health as two separate problems, checked at two separate appointments. But thyroid and PCOS overlap shows up consistently across recent research, not as a rare coincidence.
In a 2025 study , women with PCOS were compared with healthy controls and found to have significantly higher TSH and lower FT4 levels in the PCOS group. Subclinical hypothyroidism was present in 14.9 percent of the PCOS group, versus only 5.3 percent of controls.
How Common Is the Thyroid and PCOS Overlap
That 14.9 percent figure is not an outlier. A case-control study found subclinical hypothyroidism in 43.5 percent of women with PCOS, compared with 20.5 percent of controls, roughly double the rate.
In fact, a separate meta-analysis pooling multiple studies showed an overall prevalence of subclinical hypothyroidism in PCOS to be about 19.7 percent. All datasets fall in a similar range. The thyroid and PCOS overlap impacts somewhere between one in five and almost one in two women depending on the population studied.
Autoimmune thyroid disease follows the same pattern. A review in Frontiers in Endocrinology found autoimmune thyroiditis in 26.9 percent of PCOS patients, against just 8.3 percent of controls, close to a three-fold difference.
However, hyperthyroidism itself is rare in PCOS, although a 2019 Danish population study showed that women with PCOS had 2.5 times the risk of developing thyroid disease over time. The overlap between the thyroid and PCOS runs almost entirely in the hypothyroid and autoimmune direction, not the overactive one.

Thyroid and PCOS: The Shared Root Cause Is Insulin Resistance
Insulin resistance is the common thread across PCOS and PMOS phenotypes, present even in lean patients. It appears to be exactly what links thyroid and PCOS together as well.
More specifically, the meta-analysis cited above found that women with PCOS and subclinical hypothyroidism had significantly higher HOMA-IR scores and fasting insulin levels than women with PCOS and without subclinical hypothyroidism. The women whose thyroid numbers drifted were also the ones with the clearest insulin resistance, not some other, unrelated set of patients.
How Thyroid and PCOS Symptoms Escalate Together
An underactive thyroid does not simply add a second diagnosis alongside PCOS. It appears to actively worsen it. Thyroid hormone helps regulate sex hormone binding globulin, or SHBG, the liver protein that keeps testosterone and other androgens bound and inactive in the blood.
When thyroid function drops, SHBG production in the liver tends to drop with it. That single shift changes how PCOS itself behaves day to day.
The SHBG Link Between Thyroid and PCOS
Lower SHBG means more free, biologically active androgen in the bloodstream, and the hormone pattern is already high in PCOS. An underlying thyroid issue can push free androgen levels even higher for a woman already struggling with acne, excess hair or irregular cycles.
This is one of the clearest mechanistic explanations for why thyroid and PCOS symptoms so often escalate together, instead of staying independent of one another.
Thyroid and PCOS: Which Condition Comes First
The honest answer is that evidence currently points one direction more strongly than the other. Research support is stronger for PCOS preceding and contributing to subsequent hypothyroidism than for the reverse sequence.
That said, that doesn't rule out the possibility of thyroid dysfunction coming first in some individual cases. The better supported story is that unmanaged PCOS and its insulin resistance create conditions that make thyroid dysfunction more likely to follow. This order matters, because it changes where treatment attention often needs to start first.
What To Do If You Have Thyroid and PCOS Together
None of this evidence is a reason to worry alone. It is a reason to test properly, and to treat both conditions together instead of in isolation.
If you have a confirmed diagnosis of PCOS or PMOS, a full thyroid panel is a reasonable ask at your next visit. One TSH reading isn’t enough, considering how often the two conditions travel together in the research.
Getting Tested for Thyroid and PCOS: A Food-First Plan
Ask specifically for TSH, free T4, and thyroid antibody testing. Autoimmune thyroiditis is the form of thyroid disease most strongly linked to PCOS (see my Blog on Hashimoto's for the full testing picture). A single normal TSH reading, taken alone, can miss antibody positivity that often shows up years before hormone levels shift out of range.
On the diet side, the same food-first approach helps both conditions. Both respond to the same underlying lever, which is better insulin sensitivity.
In practice, build meals around protein and fiber, and distribute carbs throughout the day. Correcting the common Indian deficiencies in iodine and selenium also matters. Together, these address the shared metabolic root behind thyroid and PCOS, rather than treating each condition’s symptoms in isolation. A realistic full-day thyroid diet chart walks through exactly how this looks, meal by meal.

The Bottom Line on Thyroid and PCOS
Thyroid and PCOS overlap is common enough that it should be tested for as a matter of routine. It is not a surprising coincidence when it turns up on a report.
Ultimately, insulin resistance appears to be the shared mechanism driving both conditions. Correcting it through food-first care addresses more than one problem at once. The common thread here echoes what we have said before: insulin resistance is the root across PCOS and PMOS phenotypes, and thyroid health is very much part of that same picture. Frequently Asked Questions About Thyroid and PCOS
What Causes the Thyroid and PCOS Overlap?
Current evidence supports PCOS contributing to later thyroid dysfunction more strongly than the reverse sequence. Insulin resistance from PCOS appears to raise the risk of subsequent hypothyroidism, though individual cases can still vary.
How Common Is Thyroid and PCOS Overlap in Women?
Subclinical hypothyroidism occurs in about 15 to 44 percent of women with PCOS (depending on the study), versus about 5 to 20 percent of women without PCOS. Autoimmune thyroiditis is about three times more common in PCOS as well.
Can Hyperthyroidism Occur With Thyroid and PCOS Overlap?
That is unusual. Thyroid and PCOS overlap is almost exclusively hypothyroidism and autoimmune thyroiditis, not overactive thyroid. Hyperthyroidism and PCOS should still be evaluated on its own merits.
What Tests Confirm Thyroid and PCOS Overlap?
Ask for TSH, free T4, and anti-TPO antibody testing together, rather than TSH alone. Antibody positivity can appear years before TSH itself moves out of the normal range.
Does Treating Thyroid Function Improve PCOS Symptoms?
It can help. Correcting an underactive thyroid raises SHBG production, which reduces free circulating androgens, the same hormones responsible for many visible PCOS symptoms like acne and excess hair growth.
Is Insulin Resistance the Real Link in Thyroid and PCOS Overlap?
This is exactly what the current research is indicating. Women with PCOS and subclinical hypothyroidism have significantly higher markers of insulin resistance than women with PCOS alone, indicating a common metabolic origin.
Does Weight Loss Fix Thyroid and PCOS Together?
Weight loss can help symptoms in women carrying excess weight. But it is not a cure for either condition, and it does not apply to lean PCOS, where insulin resistance exists without excess weight.
What Is the Best Diet Change for Thyroid and PCOS Together?
Begin with protein and fiber in your anchor meals and then spread your carbs throughout the day. This supports the insulin sensitivity that both thyroid and PCOS seem to need for any real improvement.
